Legal U.S. resident; outpatient; valid prescription from a licensed U.S. health care professional for a product included in the program; no coverage for the requested Bausch Health product or denied/exhausted coverage options; household income at or below 300% of the Federal Poverty Level. Applicants may need insurance cards, pharmacy out-of-pocket cost documentation, income documentation, and proof of U.S. residency if they do not have a Social Security number.
