Who this is for
Georgia residents with an approved intellectual or developmental disability (developmental disability, intellectual disability, cerebral palsy, autism spectrum disorder, or Down syndrome) living at home with family who do NOT receive a NOW/COMP Waiver. Apply with a completed Application for Family Services, a signed Family Support Agreement, and documentation of the diagnosis (psychological evaluation, school IEP, physician verification, or DD intake assessment).
How to apply
Apply through a local community-based Family Support provider; complete the Application for Family Services and Family Support Agreement and submit diagnosis documentation. Contact a DBHDD regional field office to find your local provider.
