Georgia residents age 4 and up with a diagnosed developmental disability (intellectual disability, cerebral palsy, autism, Down syndrome, etc.) who live at home and are NOT receiving a NOW or COMP waiver; bring documentation of the diagnosis.

Apply at a local community-based provider listed by county on the DBHDD website; complete the Application for Family Services and Family Support Agreement and submit diagnosis documentation.
