Who this is for
Georgia individuals with an approved intellectual or developmental disability (developmental disability, intellectual disability, cerebral palsy, autism spectrum disorder, or Down syndrome) living at home and NOT receiving a NOW or COMP Medicaid waiver. Bring documentation such as a DD intake assessment, psychological evaluation, school IEP, physician verification, or Social Security Disability documentation.
How to apply
Complete an Application for Family Services and sign the Family Support Agreement with a local community-based provider; provide documentation of the disability. Apply through the provider list by county on the DBHDD website.
