Who this is for
Applicants must need hearing aids, be denied store financing, have exhausted other resources, have household income at or below 200% of federal poverty guidelines, and be a U.S. or Puerto Rico resident or citizen. Adult applicants age 19 and older must pay a $200 non-refundable application fee by cashier’s check or money order; children need a recent audiogram and medical clearance.
