Who this is for

Adults whose front teeth were damaged, broken, or lost because of intimate partner violence and/or sexual assault; must be out of abusive relationships or situations for at least one year, have support, stable housing, reliable transportation for appointments up to 200 miles away, no prior treatment attempt for those injuries, and not be in active drug/alcohol addiction or receiving addiction treatment.

How to apply
