Must have a prescription for an eligible Takeda medicine from a U.S.-licensed doctor, live in the U.S. or a U.S. territory, have no insurance or not enough insurance for the medicine, meet income rules generally at or below 5 times the Federal Poverty Level, and lack other coverage or funding.
