Patient must have a confirmed diagnosis of inflammatory bowel disease, including ulcerative colitis or Crohn's disease, or be getting testing for IBD; be in active treatment, starting treatment in the next 60 days, or have finished treatment in the last 6 months; be a U.S. citizen or permanent resident; and be getting treatment in the United States or a U.S. territory.
