Must live in the United States or Puerto Rico, meet income rules for most medicines at or below 300% of the Federal Poverty Level, and have no insurance or no coverage for the Bayer medicine. Medicare Part D or Medicare Advantage patients may need proof of denial from Extra Help/LIS.
