Who this is for
Must be referred directly by the treating oncologist; have late-stage cancer; have at least one child age 3 to 18; be cleared by the oncologist for travel; and not have previously participated in an adult dream, wish, or similar program. Income is not considered.
How to apply
Treating oncologist completes the referral form and sends the full referral package to aleigner@jajf.org or faxes it to (404) 920-2139. JAJF contacts eligible families after the full package is received and reviewed.
