People in the United States with a clinical diagnosis of PSP, CBD, or MSA who are cared for at home, have not received this grant before, receive no more than 20 hours per week of professional respite care, have combined annual income under $90,000, and do not have long-term care insurance. A physician diagnosis verification form is required.
