ACA, PeachCare, Medicaid, P4HB, and Medicare Application AssistanceProgram

Staff help you find and apply for health insurance. This includes the ACA Marketplace, Medicaid, PeachCare for Kids, Planning for Healthy Babies, and Medicare.

Adult Medicine and Family PracticeService

Care for adults 18 and older that focuses on staying well and managing long-term conditions such as diabetes and high blood pressure. Visits can also cover physicals, cancer screenings, injuries, ADHD treatment and prescriptions.

Behavioral Health CounselingService

Counseling for individuals, families and groups dealing with anger, anxiety, stress, grief or depression. Visits can be one time or ongoing, and telehealth visits are offered.

Care Coordination, Senior Care and Chronic Disease SupportProgram

Staff help patients follow through on cancer screening, heart health, nutrition and chronic disease needs, and connect them to specialists. For seniors, they help with medicine, Medicare enrollment information, Welcome to Medicare exams and yearly wellness visits.

HIV and PrEP SupportProgram

The team gives information, classes and referrals, and coordinates care for people living with HIV, substance use concerns or mental health disorders. What you share is private. Help may include counseling, dental care, food, housing, medicine, rides and utilities.

HIV, hepatitis and STI testingService

Testing for HIV, chlamydia, gonorrhea, syphilis and hepatitis B and C, plus hepatitis B vaccine and PrEP for HIV prevention. Family planning services, including this testing, are offered at the Cobb center.

LaboratoryService

Blood work is done on-site at the center through a partnership with Quest Diagnostics. Routine results usually come back in 1 to 2 days.

Mobile Medical and Dental UnitsProgram

Three mobile units bring primary care, shots, physicals and dental care to partner schools and organizations on set days. They also work with groups to offer free COVID-19 testing, education and shots in the community.
