CCSP and ICWP Waiver ServicesProgram

Health Force is an approved provider of home care under two Georgia Medicaid waivers. CCSP helps older or disabled people stay home instead of a nursing home. ICWP gives in-home help to adults with severe physical disabilities.

Companion and Homemaker CareService

Caregivers visit to give company and practical help around the home. They can chat, share hobbies and meals, do light housekeeping, run errands, and help with rides.

Free In-Home Nursing AssessmentService

A registered nurse visits your home to learn what help you need and set up a care plan. It is free and you do not have to sign up for care afterward.

GAPP Care for Children (Georgia Pediatric Program)Program

Nurses and aides care for medically fragile children at home through Georgia's GAPP Medicaid program. Care can include nursing tasks like feeding tubes, breathing equipment, and medicines, plus help with daily activities.

Georgia Medicaid Waiver Services (CCSP, SOURCE, ICWP)Program

Health Force is a state-approved provider for Georgia's Medicaid waiver programs, which pay for home care so older adults, adults with disabilities, and medically fragile children can stay home. The programs include CCSP, SOURCE, Structured Family Caregiving, ICWP, and GAPP.

Home Health Care ServicesService

Health Force offers a full range of care at home, from a few hours a week to 24/7. Services include companion care, personal care, nursing, respite, dementia care, and IV therapy.

Home Infusion TherapyService

Nurses give IV treatments at your home, such as antibiotics, nutrition, and pain medicine.
