Who this is for
You must be a Georgia resident and a U.S. citizen with a traumatic brain injury or traumatic spinal cord injury. You need a letter or record from a doctor or medical facility that says the type, cause and date of the injury. Stroke, disease, and oxygen-loss injuries do not qualify. You should first use other help you qualify for, like Medicaid, Medicare or insurance. Bring one price quote for each item, a notarized citizenship affidavit, and proof you live in Georgia.
How to apply
Apply online, or download and mail the paper application. Include your injury papers, a price quote for each request, a notarized citizenship affidavit, and proof of Georgia residence. Staff check that it is complete, then a committee reviews it, then the Commission votes, then the Governor's Office approves it. You get a letter with the decision. After an award, the vendor you pick must be approved as a state vendor before it can be paid.
