You must have a traumatic brain injury, or you may be a family member, caregiver, or community member who wants help. You also need to show proof of the brain injury.
How to apply
Fill out the online Contact Us form that gives permission to take part and confirms the brain injury. Also complete the privacy (HIPAA) form, then submit. Staff will contact you.
