You may use this if you have blindness or vision loss that cannot be fixed with contacts or glasses. You also need a dilated eye exam from the last 2 years to set up a low-vision evaluation.
How to apply
Fill out the Application for Services. Give the doctor forms to your eye doctor, who faxes records to VRS. Send your part by mail, email, or fax (fax 770-432-5457). VRS then calls you to set up the appointment.
