Who this is for
People whose vision loss cannot be fixed with glasses or contacts, with no other medical or surgical options. You need a dilated eye exam within the past 2 years. There is no evaluation for people who are totally blind. Bring any magnifiers, glasses or glare shades you use.
How to apply
Fill out the Application for Services and send it to VRS by mail, email or fax. Your doctor also sends their reports. VRS then calls you to set up the appointment.
