Who this is for
You can use this if you are in active breast cancer treatment (surgery, chemotherapy or radiation) after a diagnosis confirmed by a biopsy. You or your spouse or partner must have been working when you were diagnosed, and your household income must have dropped because of it. Your household income must be at or below 500% of the federal poverty level, based on last year's federal tax return. You must live, work, file federal taxes and get treatment in the United States. You will need tax returns, W-2s, pay stubs, bank statements, a letter from your employer, medical forms, bills, and your rental agreement if you rent.
