based on income, and also offers grants to patients who meet particular eligibility requirements. To access our services, you will need to be assessed by our Patient Navigator. The Patient Navigator assists patients with information and education regarding access to healthcare and the patient resources that are available. The Patient Navigator can provide regular contact with newly diagnosed patients through the initial medical service enrollment period and can offer emotional support and make referrals if needed, to support groups and workshops.
