Who this is for
You must be uninsured, live in the 20-county Kaiser metro Atlanta area, and have household income at or below 100% of the federal poverty level (about $15,960 for one person in 2026). You cannot be eligible for Medicaid, PeachCare, Medicare, affordable job-based coverage or federal subsidies. Children must be under 26. You must be enrolled with a participating community partner. Papers: Kaiser application and subsidy form, a community partner letter, proof of income if it applies, and a Medicaid denial letter for children 18 and under if it applies.
How to apply
Apply during open enrollment through Zion Hill CDC as your community partner. Complete the Kaiser application and subsidy form and bring the other papers. Email kaiser@zionhillcdc.org with questions.
