Who this is for
You can use this if you live in the U.S. or a territory, your household income is at or below 300% FPL, and you have never gotten a Komen financial assistance award. You must be in active treatment for stage 0-3 breast cancer diagnosed within 24 months, or you must live with stage 4 breast cancer. You also need household income information and a provider-signed note on official letterhead dated within 12 months that shows your diagnosis, stage, and treatment.
