The goal of MAP is to provide people the opportunity to take better care of themselves by offering them the education and medical supplies they need to stay healthy.This program provides: - Diabetes medication- Diabetes supplies - Health EducationUpon approval, the client may receive up to 3 months of assistance for diabetes medicines and supplies.To apply for this program please download and complete the MAP application and contact us via email with any questions.
