Who this is for
Adults 18 or older with Medicare drug coverage (Part D) whose plan does not already cover these drugs for them, and who meet a BMI rule: BMI 35 or higher, or BMI 30 or higher with certain heart failure, hard-to-control high blood pressure or kidney disease, or BMI 27 or higher with prediabetes, a past heart attack or stroke, or blocked arteries in the legs or arms. Not for people with type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease.
How to apply
Talk with your provider. They send a prescription for a covered drug to your pharmacy and complete a prior authorization if asked. You get a letter from Medicare when it is approved.
