Who this is for
Men 18 or older who are certified as homeless, have a diagnosed substance use disorder, and are in a licensed medical detox facility when they are referred. The intake form needs a mental health evaluation, a TB test, a syphilis (RPR) test, a recent physical exam and the case worker's contact information.
