Who this is for
You may use this if you are a child, youth, or young adult with a main diagnosis tied to an acquired brain or spinal cord injury. The injury or diagnosis must be on or after January 1, 2011 and before your 21st birthday. You must be a U.S. citizen, be sent by a social worker, caseworker, nurse, doctor, child life specialist, or other medical professional, and have gotten treatment in the metro Atlanta area, including Children's Healthcare of Atlanta, Shepherd Center, Emory, or Grady.
How to apply
Read the grant requirements and download the application from the website. Fill it out with the medical and cost papers attached. Send it by email to Ali Smith or by mail to the PO Box. If chosen, the team works with your providers and pays them directly.
