Child, youth, or young adult with a primary diagnosis related to an acquired brain or spinal cord injury; injury or diagnosis on or after January 1, 2011 and before the patient's 21st birthday; U.S. citizen; referred by a social worker, caseworker, nurse, doctor, child life specialist, or other medical professional; received treatment in the metro Atlanta area, including Children's Healthcare of Atlanta, Shepherd Center, Emory, or Grady.
