Who this is for
Your child must be 21 or younger, or 22-25 if getting pediatric treatment. A doctor must care for the child, the insurance must not pay for the request, and the family income must be below the foundation's limit. You can ask once every 12 months, and up to 3 times in a child's life.

Read all the rules, then apply for a grant. Send a doctor's letter, income proof, a price quote from the provider or seller, and a letter showing insurance does not cover it. The foundation contacts you if it needs more.
