Who this is for
Must live in the U.S. or a U.S. territory and have a valid prescription from a U.S. licensed prescriber. Must be uninsured, underinsured, or in significant financial hardship despite insurance. Total household income before taxes cannot be more than 500% of the Federal Poverty Limit. Not for clinics or hospitals. People on government health programs such as Medicaid or Medicare cannot get CIMZIA through this program. Bring or include copies of insurance cards if insured.
How to apply
The patient fills out the patient section. The doctor fills out the prescriber section and adds the prescription. Send it online, by fax, by email, or by mail. Include copies of insurance cards, or say you are uninsured and expect a call to confirm.
