Who this is for
Georgia residents with a dog that already has a diagnosis and a vet treatment plan. The owner must show proof of income or proof of a government benefit such as SSDI, SSI, unemployment, public assistance, Medicare or Medicaid. Household income above 250% of the federal poverty level does not qualify. The owner must also pay part of the cost. Bring a vet invoice or estimate (bill of $1,200 or less), proof of income or benefits, and a clear photo of the dog.
