Care Management and Life Care PlanningService

A licensed care manager works with the family to plan and coordinate a loved one's care. This covers health care, medicines, transportation, recreation, and daily needs. The care plan is updated over time.

Financial Management and Public Benefits HelpService

PLAN helps clients manage money and apply for benefits like Social Security, Medicaid, food stamps, and reduced-fare transit. It can also act as a Social Security representative payee.

Health and Mental Health CoordinationService

PLAN's care managers guide clients through the health and mental health system. They watch medications and mental status and work with doctors, hospitals, psychiatrists, counselors, and attorneys.

Housing Search SupportService

PLAN does not provide housing. Its care managers help clients look for a living situation that fits their needs, using public and private resources.

Supportive ServicesService

This program helps people with mental illness or developmental disabilities and their families get support for housing, transportation, care, forms, and daily living. It is for adults 18 and older, including people with all disabilities, individuals, families, caregivers, and people with mental health needs.

Trust Advocacy and Special Needs Trust SupportService

If a loved one is the beneficiary of a trust, PLAN's care managers help the trustee understand the person's needs and how to use trust money for their care. PLAN can also connect families with attorneys and professional trustees to set up a Special Needs Trust.
